Intuitive Surgical beat second-quarter 2026 consensus on adjusted earnings and revenue, but ISRG fell from $402.40 two minutes before the 4:05:54 p.m. ET release to $371.90 at the ten-minute reaction mark, a 7.58% drop. From the $389.00 event-minute price, the decline was 4.396%. The first rebound was small: by 4:20 p.m. ET, ISRG was still 4.018% below the release-minute level.

The quarter cleared headline expectations

The company reported adjusted earnings of $2.80 a share against a $2.51 expectation and revenue of $2.89 billion against $2.82 billion expected. Intuitive's July 16, 2026 earnings release confirmed that revenue rose 19% from $2.44 billion a year earlier and non-GAAP earnings increased to $2.80 a share from $2.19.

The operating figures were also positive on their face. Worldwide da Vinci and Ion procedures increased approximately 16%, with da Vinci procedures up approximately 15% and Ion procedures up approximately 36%. Intuitive placed 468 da Vinci systems, including 246 da Vinci 5 systems, and ended the quarter with 11,710 da Vinci systems installed.

Procedure guidance stayed where it was

Intuitive's July 16, 2026 release kept the 2026 worldwide da Vinci procedure-growth outlook at approximately 13.5% to 15.5% and said growth was expected closer to the midpoint. That was the same range published in the company's April 21, 2026 first-quarter release. The company did not cut the range, but it also did not raise it after the second-quarter beat.

The July 16 release raised the non-GAAP gross-margin range to 68.0% to 69.0% from the 67.5% to 68.5% range published on April 21, while expected non-GAAP operating-expense growth narrowed to 11% to 13% from 11% to 14%. The release therefore did not present a simple guidance cut or a weak quarter. It presented strong reported figures, an unchanged procedure-growth range and better profitability assumptions, followed by an immediate negative price response.

ISRG sold first and only partly recovered

ISRG was $389.00 in the release minute and $378.51 one minute later. At 4:15 p.m. ET, the stock was $371.90, down 4.396% from the release-minute level and 7.58% from the price two minutes before the headline. At 4:20 p.m. ET, it recovered to $373.37, narrowing the release-minute loss to 4.018%. The move faded modestly between ten and fifteen minutes, but most of the initial repricing remained.

Intuitive's July 16, 2026 release scheduled the earnings call for 4:30 p.m. ET, after the 4:05-to-4:20 p.m. ET price path. Later management remarks therefore cannot be treated as information that drove the first move. The opening tape established a narrower point: the headline beats were not sufficient to produce a positive reaction.

Later reporting shifted attention to U.S. procedure demand

Reuters reported on July 17, 2026 that U.S. procedure growth slowed to 12% from 14% in the first quarter and that management discussed changes in patient coverage and premiums that could affect when patients seek treatment. Those details sharpened the subsequent demand debate, but they followed the initial release window and should not be backfilled as the cause of the first fifteen-minute selloff.
